Transfers administration of the Adult Reentry Grant Program from the Board of State and Community Corrections to the Department of Housing and Community Development, effective July 1, 2026, to provide 5-year renewable grants to regional administrators for permanent supportive housing and reentry services for formerly incarcerated individuals

Establishes eligibility for individuals scheduled for release from prison within 210 days who are likely to become homeless, or those currently homeless on parole or postrelease community supervision who were incarcerated within the last five years

Requires program funds to be used for rental subsidies (up to two times fair market rent), operating subsidies for affordable housing, landlord incentives, voluntary multidisciplinary services including case management, employment support, and housing navigation services following the Housing First model

Mandates the Department of Corrections and Rehabilitation to assess inmates' risk of homelessness at least 210 days before release, asking about housing plans and history of homelessness

Requires regional administrators to report annually on participants served, housing outcomes, and recidivism rates disaggregated by race and gender, with an independent evaluation due to the Legislature by July 1, 2030
