Requires health care service plans, health insurance policies, and self-insured employee welfare benefit plans issued, amended, or renewed on or after January 1, 2028, to cover screening mammography and diagnostic breast imaging without cost sharing (no copays, deductibles, or coinsurance).

Covers diagnostic breast imaging following abnormal mammography results and for individuals with breast cancer risk factors, including family history, known genetic mutations, or dense breast tissue.

Includes coverage for breast MRI, breast ultrasound, contrast-enhanced mammography, molecular breast imaging, and other clinically indicated diagnostic testing consistent with National Comprehensive Cancer Network Guidelines.

High deductible health plans must cover these services without cost sharing only after the deductible is satisfied, except for items qualifying as preventive care under federal tax code Section 223(c)(2)(C).

Plans must arrange out-of-network services if diagnostic breast imaging is unavailable within the network to ensure timely access to care.
