Prohibits health care service plans and health insurers licensed in California from discriminating against enrollees or insureds based on race, color, national origin, age, disability, or sex.

Defines sex-based discrimination to include discrimination based on sex characteristics (including intersex traits), pregnancy, sexual orientation, gender identity, and sex stereotypes.

Bars insurers from denying, limiting, or imposing additional costs on health care services based on an individual's sex assigned at birth, gender identity, or gender otherwise recorded, including gender transition and gender-affirming care.

Requires health plans and insurers to include nondiscrimination notices in coverage documents, along with information on how to file grievances regarding discrimination.

Preserves insurers' ability to deny coverage for legitimate, nondiscriminatory clinical reasons such as medical necessity, provided decisions are not based on unlawful bias or used as pretext for discrimination.
