Requires the Department of Housing and Community Development to develop and publicly issue a statewide homelessness prevention strategy by July 1, 2027

Mandates homelessness prevention action plans for 12 state agencies, including the Department of Corrections and Rehabilitation, State Department of Social Services, Employment Development Department, and State Department of Education

Defines "at risk of homelessness" as individuals with risk factors including prior homelessness experience, annual income below 30% of area median income, or rent burden

Requires the department to develop evidence-based model prevention practices, including use of predictive models to identify at-risk individuals, financial assistance programs, eviction defense services, and case management

Requires annual review and update of the strategy, along with accountability measures and evaluation of the effectiveness of prevention programs on reducing homelessness
