Establishes a $0.01 copayment for nonemergency emergency room services for Medi-Cal expansion adults (income 100-138% of federal poverty level), effective no sooner than October 1, 2028, with providers able to collect, retain, or waive the amount

Preserves 3 months of retroactive Medi-Cal coverage eligibility prior to application date using state funds where federal funding is reduced, seeking federal reimbursement to the maximum extent allowable

Exempts from copayment requirements: emergency services, family planning, services for individuals under 21, pregnant individuals, inpatients, hospice patients, primary care, mental health, substance use disorder services, and any service with Medi-Cal payment of $10 or less

Requires the Medi-Cal application to be user-tested for accuracy and readability and to incorporate work or community engagement requirements, allowing self-attestation for compliance with those requirements to the extent permitted by law

Caps total family cost-sharing at 5% of family income applied monthly, and prohibits providers from denying care solely due to nonpayment of copayments
