Requires Medi-Cal managed care plans to make credentialing determinations within 90 days of receiving a completed application, with provisional approval granted for 120 days if the deadline is missed

Mandates that Medi-Cal managed care plans use the Council for Affordable Quality Healthcare (CAQH) standardized credentialing form beginning January 1, 2028

Provisional approval does not apply if the provider is subject to licensing discipline, has adverse action reports or malpractice payments in the National Practitioner Data Bank, or has not been credentialed by the plan in the past five years
