Requires the California State Department of Public Health to establish minimum immunization rates needed to prevent the spread of communicable diseases at schools and childcare facilities

Mandates that schools and institutions notify parents or guardians when immunization rates fall below the thresholds established by the Department of Public Health

Applies to public and private elementary and secondary schools, childcare centers, day nurseries, nursery schools, family daycare homes, and development centers

Notifications must include both the required immunization rate and the actual rate for each disease that falls below the threshold

Provides for state reimbursement to local agencies and school districts if the Commission on State Mandates determines the requirements impose state-mandated costs
