Creates the Parkinson's Disease Research Voluntary Tax Contribution Fund, allowing California taxpayers to designate contributions on their tax returns in excess of their tax liability

Continuously appropriates fund revenues to the State Department of Public Health to support the Richard Paul Hemann Parkinson's Disease Program, which collects data on Parkinson's disease incidence in California

Requires the Franchise Tax Board to add the fund to tax return forms when another voluntary contribution is removed or space becomes available

Sets a minimum annual contribution threshold of $250,000; the fund will be repealed if contributions fall below this amount in any calendar year after the second year

Automatically sunsets on January 1 of the seventh calendar year following the fund's first appearance on tax returns, unless repealed earlier for failing to meet minimum contributions
