Appropriates $208,800,000 annually from the General Fund for fiscal years 2025-26 through 2028-29 for the Juvenile Justice Realignment Block Grant program, with a new county distribution formula phased in starting 2026-27 based on youth adjudications, facility transfers, and youth population

Prohibits county boards of supervisors from allocating juvenile justice block grant funding to any juvenile hall, camp, ranch, or secure youth treatment facility found unsuitable for youth confinement during the prior fiscal year

Requires counties to provide CalWORKs and CalFresh recipients with prepopulated semiannual eligibility reports (via mail or electronically at recipient's election) instead of blank forms, with department policy guidance due by August 15, 2025

Beginning July 1, 2026, requires counties to pay 100% of lost federal financial participation if the state loses enhanced Medicaid funding for In-Home Supportive Services due to noncompliance with Community First Choice Option reassessment requirements (50/50 state-county split for 2025-26)

Authorizes Adoption Assistance Program payments for children placed in out-of-state residential treatment facilities when adoptive parents reside in that state, capped at the lesser of California's short-term residential therapeutic program rate or the other state's rate, for up to 12 months

Requires county child welfare agencies to convene child and family team meetings for children receiving family maintenance services, effective July 1, 2025
