Individual and group health care service plan contracts issued, amended, or renewed on or after January 1, 2026, must cover bariatric surgery and at least one FDA-approved antiobesity medication for the treatment of obesity

Coverage criteria for FDA-approved antiobesity medications cannot be more restrictive than the FDA-approved indications for those treatments

Health plans may apply utilization management to determine medical necessity, but must make those determinations in the same manner as for any other covered illness or condition

The bill does not apply to specialized health care service plans covering only dental or vision benefits, or to Medicare supplement contracts

The amended version removes requirements for health insurers regulated by the Department of Insurance and removes the mandate for intensive behavioral therapy coverage
