Establishes a new essential health benefits benchmark plan for California health care service plans starting January 1, 2027, contingent on federal approval from the U.S. Department of Health and Human Services

Mandates coverage for comprehensive fertility services including artificial insemination, three gamete retrieval attempts, three embryo creation attempts, cryopreservation, unlimited embryo transfers, donor sperm (two vials), donor eggs (ten), and surrogacy-related services

Requires coverage for durable medical equipment including mobility devices (walkers, wheelchairs, scooters), augmented communication devices, CPAP machines, portable oxygen, and hospital beds

Adds hearing care benefits consisting of one annual hearing exam and one hearing aid per ear every three years

Applies to individual and small group health care service plan contracts issued, amended, or renewed on or after January 1, 2027, and does not apply to specialized plans, Medicare supplements, or grandfathered health plans
