The Insurance Commissioner shall convene a stakeholder group including carriers, providers, and consumers to develop a standardized format for health benefit plans, limited benefit health insurance, and dental plans.

The standardized format shall cover section names and placement in policy forms issued by all carriers, and required information for explanation of benefits forms sent to covered persons or providers.

The Commissioner shall adopt rules after considering input from stakeholders, with the rules applying to health benefit plans, limited benefit health insurance, and dental plans issued or delivered on or after January 1, 2012.

The act takes effect 12:01 a.m. on the day following the ninety-day period after final adjournment of the general assembly (August 11, 2010, if adjournment occurs on May 12, 2010).

If a referendum petition is filed within the ninety-day period, the act shall not take effect unless approved by voters at the November 2010 general election.
