Requires all health insurance carriers to make care coordination payments to health care providers offering patient-centered medical homes for adults no later than January 1, 2014.

Defines patient-centered medical home as a health care team providing high-quality, safe, continuous, coordinated, comprehensive, and evidence-based care to adults based on core principles including enhanced access, comprehensive care, care management, cost effectiveness, quality and safety, continuity of care, and preventive whole-person orientation.

Allows carriers and public payors to make care coordination payments prior to January 1, 2014 and does not prohibit other payment reform, care coordination, or health care delivery reform developed or initiated before the bill's effective date.

Declares that Colorado's health and economic security are at risk due to rising health care costs and that comprehensive coordinated care improves outcomes and reduces waste.

Act takes effect August 10, 2011, unless subject to referendum petition, in which case it requires voter approval at the November 2012 general election.
