Modifies the State Measurement for Accountable, Responsive, and Transparent (SMART) Government Act to require state departments to prepare citizen-centric performance and accountability reports instead of standard performance reports, starting December 1, 2012.

Establishes four mandatory pages for each report: Page One includes department mission/vision, operational structure, employee count, and population served; Page Two includes key performance goals and measures; Page Three includes financial data and service costs; Page Four includes performance evaluation results and future challenges.

Requires the Office of State Planning and Budgeting to establish a single design template for all department reports, including layout, colors, graphics, fonts, and imagery to maximize visual appeal and readability.

Mandates posting of reports on state of Colorado and governor's office websites, as well as individual department websites, and distribution to all general assembly members for use in annual appropriation decisions.

Exempts certain departments from reporting requirements including the Department of State, Department of Treasury, Department of Law, Judicial Department, and Office of State Public Defender.
