Extends the period for voluntary contributions to the adult stem cells cure fund on Colorado state individual income tax return forms from tax years beginning January 1, 2011 through January 1, 2016 (previously set to expire January 1, 2011).

Requires the Colorado state individual income tax return form to include a line allowing taxpayers to voluntarily designate contributions to the adult stem cells cure fund created in section 25-40-103, C.R.S.

Delays the repeal of the voluntary contribution program from January 1, 2012 to January 1, 2017, unless the general assembly continues or reestablishes the program by bill prior to that date.

Makes the act subject to a referendum petition filed within 90 days after final adjournment; if petitioned, the act requires approval by voters at the November 2012 general election to take effect.
