Extends the voluntary contribution designation line for the Make-A-Wish Foundation of Colorado fund on state individual income tax returns from tax years beginning January 1, 2012 through January 1, 2017 (previously set to expire January 1, 2013).

Allows Colorado taxpayers to designate voluntary contributions to the Make-A-Wish Foundation of Colorado fund on their state income tax return forms during the extended period.

Delays the automatic repeal of the Make-A-Wish Foundation voluntary contribution program from January 1, 2013 to January 1, 2018, unless the General Assembly continues or reestablishes the program by bill prior to that date.

Applies emergency safety clause measures, declaring the act necessary for immediate preservation of public peace, health, and safety.
