Establishes seven forms of valid photo identification required for voting in Colorado elections: valid Colorado driver's license, state ID card, U.S. passport, government employee ID with photo, federal pilot's license, U.S. military ID with photo, and valid student ID from a Colorado public institution of higher education.

Adds Medicare or Medicaid cards as valid identification only for eligible electors who are residents of state-licensed facilities.

Removes three previously accepted forms of identification: copies of utility bills, bank statements, government checks, paychecks, or other government documents showing name and address; certified birth certificates; and certified naturalization documentation.

Takes effect January 1, 2013, and applies to all elections conducted on or after that date.

Requires submission of the act to Colorado voters for approval or rejection at the next biennial general election under the state's referendum provisions.
