Prohibits owners, employees, and operators of tanning facilities from allowing persons under 18 years of age to use artificial tanning devices unless they have a physician's written prescription or parental/legal guardian consent effective July 1, 2012.

Requires parents or legal guardians to remain on-site at all times while minors under 14 years of age use tanning devices; parents of minors 14-17 may sign a consent form valid for 6 months allowing independent use without parental presence.

Mandates the Department of Public Health and Environment develop a standard consent form detailing health and safety risks associated with artificial tanning devices, available for free download from the department's website.

Requires tanning facility owners to verify age using appropriate documentation and maintain signed consent forms on file for department inspection upon request.

Subjects facility owners, employees, and operators to penalties pursuant to section 25-5-1009 for each incident of noncompliance; excludes emancipated minors from these requirements.
