• Creates a special "Flight for Life Colorado" license plate available for motorcycles, passenger cars, trucks, and noncommercial vehicles not exceeding 16,000 pounds empty weight, with issuance beginning January 1, 2013 or when the department is able to issue them.

• Requires applicants to pay standard license plate taxes and fees plus a one-time $25 fee for plate issuance or replacement, with the additional fee credited to the Highway Users Tax Fund.

• Applicants must provide a certificate from Flight for Life Colorado confirming they donated $25 to the organization, and Flight for Life Colorado must file an annual statement verifying nonprofit status.

• Allows personalized Flight for Life Colorado license plates upon payment of additional personalization fees, with ability to transfer existing personalized plate combinations to the new plates.

• Appropriates $2,960 to the Department of Revenue for information technology services and $17,760 to the Division of Motor Vehicles for license plate purchases related to implementation.
