SB12-127 Summary

• Adds statute 25.5-6-113 establishing that Colorado's population over 65 will increase more than 23% between 2005-2015, creating demand for integrated long-term care services.

• Requires the state department to allow long-term care providers (nursing homes, home care agencies, community mental health centers, hospice centers, and others) to participate as health homes in Medicaid care coordination programs.

• Health homes must provide comprehensive care management, care coordination and health promotion, transitional care, patient and family support, referrals to community services, and health information technology integration.

• Permits dually eligible persons (eligible for both Medicaid and Medicare) to voluntarily elect participation in either Medicare coordinated care systems or the state's Medicaid coordinated care system.

• Declares the act necessary for immediate preservation of public peace, health, and safety.
