Requires all sickness and accident insurance policies to provide coverage for preventive mammography and diagnostic breast imaging for individuals with at least one breast cancer risk factor, including family history, age 40 or older, genetic predisposition, or presenting with symptoms.

Preventive mammography coverage is not subject to copayments, deductibles, or coinsurance; diagnostic breast imaging coverage is not subject to cost-sharing.

Defines "diagnostic breast imaging" as medically necessary testing needed for providers to evaluate health status for early breast cancer detection.

Applies to individual and group health insurance policies, health care service contracts, and indemnity contracts delivered or issued in Colorado, with exceptions for supplemental or limited benefit policies.

Takes effect January 1, 2015, and applies to policies issued, delivered, renewed, or reinstated on or after that date; subject to voter approval if a referendum petition is filed within 90 days of final adjournment.
