Establishes a transition plan requiring youth ages 18-21 with intellectual and developmental disabilities currently served through child welfare to move to adult services by January 1, 2016.

County departments must identify and assess eligible youth by June 30, 2014, with transition plans addressing social, health, residential, educational, and guardianship needs.

Creates the Child Welfare Transition Cash Fund with an initial $2,829,586 transfer on July 1, 2014, to cover direct and indirect costs of transitioning youth to home- and community-based services.

Requires the Department to report to the Joint Budget Committee on January 1, 2015, and January 1, 2016, on transition progress by county, needs assessments, and residential placements.

Appropriates $2,829,586 from the transition cash fund to the Division of Child Welfare and $5,746,227 ($2,829,586 state plus $2,916,641 federal) to the Division of Intellectual and Developmental Disabilities for program costs.
