Authorizes the Colorado Department of Revenue to issue special EMS license plates for motorcycles, passenger cars, trucks, and recreational vehicles not exceeding 16,000 pounds empty weight, beginning January 1, 2015 or when the department is ready to issue them.

Allows the Emergency Medical Services Association of Colorado to design the plate subject to department standards and requires applicants to provide a certificate confirming a $10 donation to the association.

Charges a one-time fee of $25 for issuance or replacement of the EMS plate, in addition to regular license plate taxes and fees, with the additional fee credited to the Highway Users Tax Fund.

Permits the Department of Revenue to discontinue issuing EMS plates if fewer than 3,000 plates are issued by July 1, 2019, though existing plate holders may continue using their plates indefinitely.

Appropriates $4,120 for computer programming services and $5,920 for special license plate purchases in fiscal year 2014-2015 to implement the new plate program.
