Requires workers' compensation insurance carriers authorized to conduct business in Colorado to submit annual reports by July 1 listing all policy forms, endorsements, riders, letters, notices, and other documents in use, with officer certification that each complies with Colorado law.

Mandates carriers submit new policy forms and related documents to the Commissioner of Insurance at least 31 days before use, with officer certification of compliance with Colorado law.

Authorizes the Commissioner of Insurance to examine and investigate carriers to determine whether workers' compensation forms comply with the certification statements and statutory mandates.

Designates certifying or issuing workers' compensation forms that do not comply with statutory mandates as unfair methods of competition and unfair or deceptive acts in insurance, subject to existing sanctions.

Takes effect August 6, 2014, subject to potential referendum petition requiring approval by voters in the November 2014 general election.
