Renames the "nursing facility culture change accountability board" to the "Nursing Home Innovations Grant Board" and requires the Department of Health Care Policy and Financing to establish it by September 1, 2014

Distributes $250,000 per fiscal year from the nursing home penalty cash fund for innovation grants if the fund balance exceeds $2 million; distributes 25% of prior year deposits (up to $250,000) if balance is below $2 million

Grants must benefit nursing facility residents directly or indirectly through staff education, with projects lasting 1-3 years that are portable, sustainable, and replicable in other facilities

Changes board composition from three to four members employed in long-term care nursing facilities and removes the Colorado culture change coalition representative

Appropriates $165,000 from the nursing home penalty cash fund to the Department of Health Care Policy and Financing for fiscal year 2014-15 and extends the board's sunset date from September 1, 2016 to September 1, 2021
