Creates the interagency farm-to-school grant program administered by the Department of Public Health and Environment and the interagency farm-to-school coordination task force to provide grants to farmers and ranchers selling to schools.

Establishes two types of grants: cost-share grants for on-farm food safety investments (training, equipment, supplies, capital improvements) and cost-share grants for sales-related expenses (aggregation, transportation, production process changes, season extension).

Requires minimum annual appropriation of $500,000 to the grant program with task force authority to award grants; appropriates $500,000 for fiscal year 2015-16 to the prevention services division.

Mandates task force to issue implementation guidelines, review grant program effectiveness, and submit reports to the joint education committee on February 1, 2016, February 1, 2018, and February 1, 2019.

Repeals the grant program effective July 1, 2021, with mandatory sunset review under the Colorado sunset law before the repeal takes effect.
