Expands the "Physician Designation Disclosure Act" to include dentists, renaming it the "Physician and Dentist Designation Disclosure Act"

Requires health care entities to provide dentists with the same disclosure requirements as physicians regarding methodology, data, and criteria used for quality or performance designations

Establishes appeal procedures allowing designated dentists to challenge designations, including rights to corrected data submission, face-to-face meetings or teleconference options, and written decisions with stated reasons

Mandates conspicuous disclaimers in bold-faced type accompanying any dentist designation disclosure, stating designations are guides only and should not be the sole factor in selecting a dentist

Takes effect August 5, 2015, and applies to dentist designations on or after the effective date
