Extends the deadline for Colorado state individual income tax return forms to include a voluntary contribution line for the Colorado Healthy Rivers Fund from January 1, 2016 to January 1, 2021.

Taxpayers may continue to designate any amount they wish to contribute to the Colorado Healthy Rivers Fund on their annual income tax returns through tax year 2020.

Modifies Colorado Revised Statutes section 39-22-2402 to amend the expiration date of the voluntary contribution designation provision.

Takes effect 90 days after final adjournment of the general assembly (August 5, 2015) unless a referendum petition is filed, in which case it requires voter approval at the November 2016 general election.
