Eliminates requirement for a court order to change gender designation on Colorado birth certificates; instead requires a written request and a statement from a licensed medical or mental health care provider confirming gender transition treatment or intersex condition.

State registrar must issue a new birth certificate (rather than an amended one) that reflects the changed gender designation and does not indicate the certificate has been altered.

Original birth certificate and related records are sealed and only accessible by court order or written request of the person, except records for those under 18 years old are not sealed.

Allows name changes to be made simultaneously with, before, or after gender designation change with appropriate documentation; eliminates requirement for separate legal name change court order.

Grants Colorado courts jurisdiction to issue decrees for Colorado residents born out-of-state or abroad whose home state or country requires court involvement to amend birth certificates for gender designation changes.
