Creates new misdemeanor crime of misrepresentation of material information in birth certificate preparation, defined as knowingly and intentionally misrepresenting a birth parent's legal name, birth date, maiden name, or place of birth.

Conviction for birth certificate misrepresentation results in a fine of up to $1,000, imprisonment in county jail for up to one year, or both.

Amends existing vital statistics penalties statute to specify that the new birth certificate misrepresentation crime is the exclusive penalty for this specific offense, separate from general vital statistics document fraud.

Requires the State Registrar to revise birth certificate worksheet forms to include a statement notifying parties that knowingly and intentionally misrepresenting material information on the form is a misdemeanor.

Effective July 1, 2015, and applies only to offenses committed on or after that date.
