Directs the state department to establish a single consolidated Medicaid waiver for home- and community-based services for adults with intellectual and developmental disabilities, effective July 1, 2016, or upon federal approval.

Requires the redesigned waiver to include flexible service definitions, person-centered principles ensuring individual choice, authority over supports and services, health and safety assurances, and responsible use of public dollars.

Mandates the state department develop a conflict-free case management services plan by July 1, 2016, complying with federal person-centered planning regulations and aligning with recommendations from the Community Living Advisory Group.

Requires implementation of a functional eligibility assessment tool, person-centered assessment process with inter-rater reliability, and fair service payment system that is efficient, transparent, and equitable for providers and consumers.

Appropriates $788,347 from the intellectual and developmental disabilities services cash fund for fiscal year 2015-16 (assuming 2.7 additional FTE) and anticipates $1,388,348 in federal funds to implement the act.
