Extends the period for the Colorado multiple sclerosis fund voluntary contribution line on state individual income tax returns from income tax years 2012-2021 to income tax years 2012-2022, adding one additional year.

Changes the repeal date of the multiple sclerosis check-off program from the sixth income tax year after executive director certification to January 1, 2023, unless the general assembly continues or reestablishes the fund by bill prior to that date.

Allows taxpayers to designate voluntary contributions to the Colorado multiple sclerosis fund on their state individual income tax return forms during the extended period.

Takes effect August 5, 2015, unless a referendum petition is filed, in which case it requires approval by voters at the November 2016 general election.
