Creates a special ALS (amyotrophic lateral sclerosis) license plate for Colorado motor vehicles, available for motorcycles, passenger cars, trucks, and recreational vehicles not exceeding 16,000 pounds empty weight, beginning January 1, 2016.

Department of Revenue may stop issuing the ALS plate if 3,000 plates are not issued by July 1, 2020; existing holders may continue using their plates indefinitely.

Charges a one-time $25 fee for issuance or replacement of the ALS license plate, in addition to regular motor vehicle license plate taxes and fees; revenue goes to the Highway Users Tax Fund.

Allows applicants to personalize ALS plates with custom letters or numbers upon payment of the additional personalized license plate fee, with ability to transfer existing personalized combinations to new ALS plates.

Appropriates $5,304 to the Department of Revenue for fiscal year 2015-16: $4,120 for information technology services and $1,184 for license plate ordering.
