Requires online voter registration forms to include an option for electors to indicate whether they prefer to vote in person at a voter service and polling center or by mail ballot for all future elections.

Allows registered electors to opt out of receiving mail ballots through an electronic form, with changes taking effect if submitted at least 60 days before an election.

Requires county clerks to record ballot delivery preferences in the centralized statewide registration system and allows electors to change their preference back to mail ballots at any time.

Adds a mandatory question during voter registration asking new electors to choose between in-person voting at a voter service and polling center or mail ballot voting for all future elections.

Appropriates $62,200 to the Department of State, $56,789 to the Department of Revenue, and $16,789 to the Office of the Governor for implementation costs during fiscal year 2015-16.
