Modifies transfer amounts from the statewide death and disability plan to a member's normal retirement plan when a temporarily disabled member returns to active service or satisfies age and service requirements for normal retirement, basing the transfer on actual monthly employer and employee contributions being made to the pension plan at time of disability rather than a fixed sixteen percent cap.

Requires employers to provide newly hired members (those commencing employment on or after September 1, 1989) with a statewide standard health history form to be completed within thirty days of first employment date, with the completed form filed with the Fire and Police Pension Association within sixty days of employment commencement.

Grants service credit to members for transfers made under the modified disability restoration provisions.

Requires employers to make additional contributions to cover any difference between the transferred amount and what would have been contributed if the plan's contribution rate exceeds sixteen percent.

Takes effect August 10, 2016, unless subject to a referendum petition, in which case it requires voter approval at the November 2016 general election to take effect.
