Transfer responsibility for immunization exemption documentation from schools to the Colorado Department of Public Health and Environment (CDPHE), effective July 1, 2016.

Require parents or guardians to submit exemption certifications directly to CDPHE on a form prescribed by the department rather than to individual schools.

Direct CDPHE to post immunization and exemption rates for each school on its website, making this data publicly available.

Maintain existing exemption categories: medical contraindication based on physician certification, and religious or personal belief exemptions.

Preserve confidentiality of immunization and exemption records maintained by CDPHE under the same privacy protections as the immunization tracking system.
