Extends the Sex Offender Management Board's termination date from September 1, 2016 to September 1, 2020, implementing recommendations from the 2015 Department of Regulatory Agencies sunset report.

Requires the board to revise standards for identifying and evaluating adult and juvenile sex offenders by July 1, 2017, incorporating evidence-based risk assessment models (Risk-Need-Responsivity or similar) and identifying reliably lower-risk offenders who may not benefit from treatment.

Mandates establishment of committees with at least 80 percent approved treatment providers to revise guidelines and standards for treating adult and juvenile sex offenders, with completion by July 1, 2017.

Establishes data collection requirements for treatment providers, evaluators, and polygraph examiners to measure program effectiveness, with a data collection plan due to judiciary committees by January 2017.

Requires supervising agencies to provide offenders beginning community supervision on or after the bill's effective date with a choice of two appropriate treatment provider agencies, unless documented limitations justify fewer options.
