Requires health insurance plans in Colorado to cover annual breast cancer screening using appropriate breast imaging modality as determined by the individual's health care provider, with coverage falling within American College of Radiology guidelines.

Expands coverage to include mammograms with or without clinical exam for average-risk individuals, and mammograms plus medically recommended noninvasive imaging modalities for high-risk individuals or those with incomplete mammogram results.

Prohibits cost sharing for covered breast cancer screenings when an imaging modality is recommended by a health care provider and falls within appropriate use guidelines established by the American College of Radiology.

Requires coverage for individuals with at least one breast cancer risk factor, including family history, age 40 or older, symptoms, or genetic predisposition determined by risk assessment models such as Tyrer-Cuzick, BRCAPRO, or Gail.

Takes effect January 1, 2018, subject to referendum petition provisions requiring voter approval in November 2016 if challenged within 90 days of legislative adjournment.
