Requires all workers' compensation insurance carriers authorized in Colorado to submit annual reports to the Commissioner of Insurance by July 1 listing policy forms, endorsements, riders, letters, notices, and other documents, with officer certification that documents comply with Colorado law.

Requires advisory organizations and rating organizations to submit annual reports listing policy forms with officer certification of compliance with Colorado law, with "form" defined to include endorsements, riders, letters, notices, and other documents affecting policies.

Allows carriers using unmodified forms from advisory or rating organizations to notify the Commissioner of adoption instead of submitting separate certification, but requires full submission if carrier modifies or uses other forms.

Requires carriers, advisory organizations, and rating organizations to submit new or revised policy forms to the Commissioner at least 31 days before use, with officer certification of compliance; exempts forms certified by advisory or rating organizations if used in entirety by member carriers.

Authorizes the Commissioner to examine and investigate carriers to determine whether policy forms comply with carrier certification and statutory mandates.
