Amends the definition of "durable medical equipment supplier" to include entities that bill or plan to bill the state medicaid program, in addition to Medicare billing entities.

Excludes suppliers that only provide insulin infusion pumps or diabetes testing supplies from the definition of durable medical equipment supplier.

Requires each physical location of a durable medical equipment supplier operating in Colorado to be licensed by the Secretary of State and maintain accreditation as a DMEPOS facility.

Mandates suppliers attest that each physical location is within 100 miles of any Colorado-resident Medicare beneficiary or Medicaid recipient being served, and prohibits using warehouses or repair facilities to meet this requirement (except for fully accredited facilities domiciled within such facilities).

Appropriates $10,300 to the Department of State for the 2017-18 fiscal year to implement the act through information technology services.
