Establishes the "2017 Birth Certificate Modernization Act" allowing Colorado residents to obtain a new birth certificate reflecting a different gender designation without court order.

Requires written request from the person (or parents/guardian if minor) and a statement from a licensed medical or mental health provider confirming the person has undergone gender transition treatment or has an intersex condition.

New birth certificate supersedes the original as the official public record and shall not be marked as amended or indicate any prior changes to gender designation or name.

Repeals prior law requiring court order indicating sex change by surgical procedure and separate legal name change order.

Allows Colorado courts to issue decrees amending birth certificates for residents born in other states or jurisdictions that require court orders for gender designation changes.
