Establishes the Accountable Care Collaborative as a Medicaid care delivery system with goals to improve member health and reduce costs through primary care medical homes, care coordination, behavioral health integration, and telehealth services.

Requires the state department to continue providing care delivery through the Accountable Care Collaborative using innovative care models and capitated managed care options.

Mandates annual reporting to the Joint Budget Committee and relevant legislative committees on enrollment numbers, performance results, administrative costs, fiscal performance, and efforts to reduce waste and inefficiencies.

Authorizes performance-based provider payments for primary care providers, federally qualified health centers, long-term care providers, and behavioral health providers based on quantifiable performance measures and quality of care standards.

Requires the state department to submit performance-based payment proposals to the Joint Budget Committee by November 1 prior to implementation, including evidence of budget savings or cost requests, stakeholder engagement summary, and projected impact analysis.
