Requires health insurers to develop written standards for selecting providers and placing them in tiered networks, with standards applied consistently by the carrier and its intermediaries.

Prohibits insurers from discriminating against high-risk populations by excluding or tiering providers based on geographic location of high-risk populations or because they treat high-risk populations.

Mandates at least 60 days' written notice before terminating or tiering a participating provider, with detailed explanation of reasons and opportunity for informal reconsideration within 45 days.

Requires insurers to disclose network selection standards to the insurance commissioner, participating providers, and the public in plain language; providers may request quarterly lists of all network plans and products.

Appropriates $42,006 to the Division of Insurance for implementation, including 0.5 FTE for personal services; applies to contracts issued or renewed on or after January 1, 2018.
