Medical exemptions may be provided as a letter (not requiring a specific form) from a licensed physician, physician assistant, or advanced practice nurse documenting that immunizations would endanger the student's health or are medically contraindicated

Religious or personal belief exemptions may be submitted as a letter (not requiring a specific form) signed by a parent, guardian, emancipated student, or student age 18 or older

Parents, guardians, physicians, physician assistants, advanced practice nurses, and students are not required to use forms furnished by the state board of health, department of public health and environment, or other entities

State board of health rules regarding exemption submission frequency are modified to address general exemptions rather than specific exemption forms

Bill takes effect August 9, 2017, unless subject to a referendum petition, in which case it requires voter approval at the November 2018 general election
