Requires abortion providers to inform women at least 24 hours before an abortion procedure about the gestational age, anatomical characteristics, physical and psychological risks, alternatives to abortion, and available financial assistance.

Mandates that women be offered an ultrasound showing the entire body of the unborn child with oral description and heartbeat (if present), with choice between abdominal or vaginal ultrasound; providers must offer free ultrasounds or provide list of free providers within 10-50 mile radius.

Requires women to sign a receipt of information acknowledging they received complete disclosures, or sign a decline form if they refuse the information; women may decline to view ultrasound images or hear fetal heartbeat.

Establishes that physician violations constitute unprofessional conduct and unclassified misdemeanor with fines up to $1,000 per violation; allows civil lawsuits for damages, punitive damages, and treble damages against providers and facilities.

Authorizes Attorney General to establish policies for collecting receipts, conducting inspections of abortion clinics, and imposing civil penalties up to $1,000 per violation or facility suspension/closure.
