Eliminates the requirement for a court order to change gender designation on a Colorado birth certificate; requires only a signed written request and medical/mental health provider statement instead.

State registrar shall issue a new birth certificate (not an amended one) that reflects the changed gender designation and supersedes the original record without indicating it was amended.

Medical or mental health provider statement must confirm the person has undergone surgical, hormonal, or other gender transition treatment per contemporary medical standards, or has an intersex condition.

Allows legal name changes to be reflected on the new birth certificate if documentation is provided before, during, or after the gender designation change.

Grants Colorado courts jurisdiction to issue decrees amending birth certificates for gender changes for state residents born in other states or foreign countries that require court orders for such changes.
