Requires hospital off-campus locations to obtain and use unique National Provider Identifiers (NPIs) separate from the hospital's main NPI on all reimbursement claims submitted on or after January 1, 2020.

Defines "off-campus location" as a facility owned or controlled by a hospital, located more than 250 yards from the main campus, providing outpatient preventive, diagnostic, treatment, or emergency services that are organizationally integrated with the hospital.

Requires organization health care providers and their subparts to obtain unique NPIs for each service site and provider type on all claims for state medical assistance programs starting January 1, 2020 for new enrollees and January 1, 2021 for revalidating providers.

Aims to increase transparency and accountability in health care costs by ensuring off-campus locations and subparts have separate billing identifiers, allowing consumers and policymakers to track costs by facility location.
