• Establishes community transition services and supports program enabling eligible persons to transition from institutional settings (nursing homes, etc.) to home- or community-based settings with ongoing services and support.

• Requires participants to be Medicaid-eligible, express willingness to transition, work with case management agencies, and reside in compliant home- or community-based settings; legal guardians must be involved in decision-making.

• Authorizes state department to seek federal waivers or state plan amendments necessary for implementation and directs state board to adopt rules including service unit limits based on available appropriations.

• Requires annual reporting to legislative committees starting November 1, 2019, detailing cost-effectiveness, number of persons requesting/receiving services, successful transitions, and persons who returned to institutional settings.

• Appropriates $215,190 in general fund and $306,000 to department of local affairs for housing subsidies for fiscal year 2018-19; anticipates $485,190 in federal funds; effective July 1, 2018.
