Prohibits training use of Class B firefighting foam containing intentionally added PFAS chemicals beginning August 2, 2019, with civil penalties of up to $5,000 for first offense and $10,000 for repeat offenses.

Bans manufacturers from selling Class B firefighting foam with intentionally added PFAS chemicals in Colorado beginning August 2, 2021, with exemptions for federal requirements, fuel storage facilities, chemical plants, and the Eisenhower-Johnson Tunnels.

Requires manufacturers and sellers of firefighting personal protective equipment to provide written notice to purchasers if equipment contains intentionally added PFAS chemicals, with records retained for three years.

Mandates the Department of Public Health and Environment conduct triennial surveys of fire departments to track possession and use of PFAS-containing foams and report findings to legislative committees by January 1, 2020.

Appropriates $55,278 to the water quality control division for 2019-20 fiscal year ($49,910 for personnel services and $5,368 for operating expenses) to implement the act.
