Requires the Department of Public Health and Environment to develop standardized forms and submission processes for medical exemptions and religious or personal belief exemptions to immunizations by January 1, 2020.

Medical exemptions must be completed by licensed physicians, physician assistants, or advanced practice nurses on a standardized form; religious or personal belief exemptions must be submitted in person to the Department or local health agency and signed by a representative before submission to schools.

State Board of Health must adopt rules requiring Hepatitis A, Rotavirus, and Meningococcal immunizations based on CDC's Advisory Committee on Immunization Practices (ACIP) recommendations.

Parents and guardians claiming exemptions must be informed of the option to exclude student immunization information from the state immunization tracking system, and forms must not require identification of religious faith.

Department must develop educational materials about immunization benefits for healthcare providers and present immunization exemption statistics annually to the General Assembly.
